IDS ::: Orthotics ::: Reciprocating Gait

RGO's (Reciprocating Gait Orthoses)

RGO's are effectively HKAFO's (Hip Knee Ankle Foot Orthoses) which have locking knee joints as normal. But, where the hip joints are able to be flexed and extended while interconnected, the connection ensures that if one hip joint is flexed the other extends.

These are almost always used by children who have little or no lower limb power, but reasonable trunk and upper limb control. Spina Bifita and spinal cord injuries are the most common conditions.

They are used with a walking frame. By weight bearing on one limb and leaning back the hip joint is effectively extended. The linkage flexes the contralateral hip joint as the contralateral foot is not weight bearing. Weight is then transferred to this side and the process is repeated. As for HKAFO's, RGO's, are normally discontinued as the patient gains length and weight at early adolescence.

RGO's have modified interconnected hip joints. These joints can be unlocked to allow flexion for sitting just like HKAFO's. However, when locked the hip joints are connected by strong cables or a rocking bar. The connection is made in such a way that flexion of one joint will cause extension in the other. Ground clearance to allow flexion/extension is created by tilting to one side slightly, while supported by a rollator or similar frame.

Dual cable RGO

While in this position, the patient leans back slightly, effectively extending the “weight bearing” hip. The connection simultaneously flexes the contralateral hip to which the patient then transfers weight. The process is repeated for each step.

RGO's require a certain amount of balance and upper body strength to be safe and effective. With growth, power to weight ratios generally reduce and the centre of gravity is raised.
